Our Vaccine Consent Forms are listed below in a PDF format. To complete the form, download the appropriate file, print the form and fill in the highlighted sections. Please complete a separate form for each pet. Having your paperwork completed prior to your arrival, expedites the check in process so your pet can be seen by the veterinarian sooner.
Please note our recommendations for your pets are for the services that
CSRA Life Saver offers and does not include all the recommendations
for services your pet may need.
Please check with your full service vet for services other than what we offer,
for example, blood work, allergy testing, skin issues, dental services, etc.
Pets should also get a full physical exam with blood work when they reach the age of six (6) years old, and then once every year to screen for senior health issues such as arthritis, glaucoma, thyroid issues, etc.

We are not a full service veterinarian and cannot treat or vaccinate sick
or injured animals; therefore, we strongly recommend
developing a rapport with a full service veterinary clinic for emergencies.

Standard medical procedures for puppies over 4 months and adult dogs:

Standard medical procedures for puppies under the age of 4 months:
- Wellness Exam
- DHPP (given every 3-4 weeks)
- Bordetella vaccine
- De-wormer (given every 3-4 weeks)
- Heartworm preventative
- Flea preventative
- Any other meds as needed
Standard medical procedures for kittens over 4 months and adult cats:

Standard medical procedures for kittens under the age of 4 months:
- Wellness Exam
- FVRCP (given every 3-4 weeks depending on age)
- De-wormer given every 3 – 4 weeks (depending on age)
- FIV/FLk (FIV & Feline Leukemia) test (Age 12 weeks of age & above)
- Feline Leukemia vaccines (if tests are negative – given every 3-4 weeks [2 vaccines])
- Flea preventative
- Any other meds as needed
